2021-2022 Grants Application
This year, for your convenience, we have several options to submit your grants application: digital, email, or mail/delivery.
There are two different grant applications available: a simplified application, and a full application.
A simplified grant application procedure is available to those applicants seeking grants of $25,000 or less. The simplified procedure is also applicable for applicants whose request exceeds $25,000 for the purchase of personal property only (e.g., lifesaving equipment).
The full grant application procedure is for those seeking more than $25,000 (not for personal property).

2021-2022 Grants Program Workshop
Our 4th Annual Community Grants Program workshop and Q&A forum was held on Tuesday, June 8 from 10-11 a.m. via zoom.
This workshop is an opportunity for local nonprofit organizations focused on health and healthcare to learn about our grants program and policy updates ahead of our FY 2021-22 funding cycle.
