2023-2024 Sponsorship Application

This form is used to request District sponsorships of a charitable event benefitting an organization providing services within the District. Sponsorship funding shall be determined based on the cost of holding the events in comparison to the funds benefitting community health programs. You may submit a sponsorship application in conjunction with or separate from a grant application.
